Cost of Service: I am in network with BCBS, UHC, UMR,  TriWest, & Aetna. All others, I can submit claims on your behalf. Members are responsible for any applicable deductible and copays.

AHCCCS and Medicare are NOT accepted

If you do not have insurance or choose direct payment for another reason, I will be glad to discuss fees at the time of contact. I offer a sliding fee scale for those who may need financial assistance.
